Meet the Team

Dr. Britt Schloemer, DNP, APRN

Dr. Britt Schloemer, DNP, ARPN started Trifecta Medical Group with the goal of providing exceptional care in a friendly and caring atmosphere where all patients can feel valued and heard.   She envisioned a practice where establishing and nourishing a trusting relationship with patients and clients is valued over profit-driving policies as are seen with large corporations.  Her ultimate goal is for Trifecta Medical Group to grow to be a place where patients of all ages can obtain care for their bodies, minds, and spirits – the trifecta-winning combination.

Britt has lived in Louisville, KY nearly her whole life.  She is a graduate of St. Leonard, Sacred Heart Academy, University of Dayton (B.S. in Biology and Minor in art), Bellarmine University (BSN & DNP), and Vanderbilt University (MSN-APRN). After working in the Neonatal Intensive Care Unit (NICU) for five years, Britt went back to school to become a Pediatric Nurse Practitioner.  Soon after graduation, she worked in Pediatric Neurology, gaining 10+ years of experience diagnosing and treating seizures, developmental delay, cerebral palsy, etc. She also was a Professor of Nursing at Bellarmine University for 8 years, teaching both undergraduate students and nurse practitioner students. Britt brings extensive knowledge and experience in developing children, the brain, and an understanding of educational best practices to best care for children and their families in primary care.  Additionally, Britt became certified in neurotoxin and fillers (cosmetic procedures) in 2021 where she fuses her knowledge and experience with the anatomy and symmetry of the face through her art background, with medicine.

Britt and her husband, J.R., live in the Highlands where their lives currently revolve around their four young children and two dogs (you may see Teddy in the office as he tries to become a therapy dog).  You can find Britt (or her husband) on any given day, shuttling children to the barn to ride horses, to the pool for swim team practice, or to the soccer field.  One day, Britt will get back to reading, hiking, traveling, and other joys outside of her family.


Dr. Brian Jochim, MSN, DNP, FNP-BC, ACNP

Dr. Jochim began his healthcare career in 2008 by graduating from Spalding University with his bachelor’s in nursing. After graduation, Dr. Jochim worked diligently as an ICU nurse before moving to the operating room. He continued to climb the career ladder by returning to school and is board certified in Family Practice as well as Acute Care Pediatrics. Finally, Dr. Jochim attended Aspen University, graduating Magna Cum Laude with his Doctorate of Nursing Practice. Within his practitioner career, Dr. Jochim has served with the neurosurgical service of Norton Childrens for over half a decade. Following this, Dr. Jochim delivered primary care to the residents of LaGrange, KY. He has recently transitioned his practice to work in recovery, fighting substance abuse within our community. Dr. Jochim lives in Louisville with his wife and two daughters. He is an avid pickleball player as well as personal fitness enthusiast. Dr. Jochim’s goal is to empower patients with the knowledge to effectively manage their health in a manner that fosters wellness both physically and mentally.


Dr. Chasity Delavega, MSN, APRN, FNP-BC 


Jax

Jax has worked in pediatrics for the last 10 years and absolutely loves it. She is currently a MA and phlebotomist, with plans to further her nursing career. She has three beautiful children, three dog fur babies, and one kittie fur baby with her husband. Jax is a die-hard New England Patriots fan and loves anything to do with skulls and Halloween. For fun, she enjoys spending time with her family, crafts, painting, and playing games.

Alisha

Alisha trained in school to become a medical assistant and cardiovascular technician in 2009 and has been working with kids ever since. She has recently gotten married and has 3 children. She enjoys painting and throwing a football with her kids.

Teddy

Chasity began her healthcare journey as a CNA in 2006, which inspired her to begin the journey of climbing the nursing ladder from CNA, to LPN, to RN, to BSN, most recently to APRN. She is alumni of Galen College of Nursing (Diploma in LPN and Associate in RN), and alumni of Indiana Wesleyan University (BSN & MSN-APRN) specializing as a Family Nurse Practitioner. Chasity has spent her nursing career working in an array of settings including long term care, acute care for adults and pediatrics, nursing clinical instruction, and correctional nursing, as well as the majority of her career in nursing management for 9 years. In 2023, Chasity completed her goal of becoming a Family Nurse Practitioner and plans to further her career in primary care. Chasity’s goal is to help all ages of patients create healthcare plans that are suitable to their own goals to improve overall health. 

Theodore Bear “Teddy” is our beloved Aussie Doodle/Bernedoodle mix and Trifecta’s official tail wagger and greeter. Teddy is training very hard to be a professional Ouchie distractor and can’t wait to help your child at their next appointment!
